OFSTED requires that all out of school clubs have specific forms for recording information on the children in your care, the activities you offer, your equipment, your premises etc.

To save you the time and effort of putting together your own forms, or tracking down individual examples to copy, we have put together a downloadable pack containing templates of all the forms you are likely to need.

When you buy the Forms Pack you will be able to download the templates straightaway. The template forms are supplied in Word format so that you can easily print them out or modify them as needed for your own childcare setting, adding your own club name, prices etc.

We have provided PDF versions of some of the forms so that you can preview them. Click on an underlined form name to see a PDF preview of that form.

To purchase the information pack online you will need a debit or credit card. If you would prefer to pay by cheque, please print out and complete the order form, and post it to us with your payment. Your information pack will be emailed to you when we receive your payment. Please ensure that you enter your email address correctly.

When you purchase the Forms Pack we will send a confirmation email to the address that you specified in the purchase process. The email will contain a link to a .zip file which contains all the templates in Word format. Click on the link to download the .zip file. Most computer systems will automatically open the .zip file for you so that you can access the Word files.
If the .zip file does not open automatically you will need to use a standard decompression utility (such as WinZip, Stuffit or PKZip) to extract the Word files.
